Director of Case Management

\*UNLIMITED PTO\* Responsibilities: - Provide leadership and oversight to the Case Management department - Develop and implement standards and protocols for case management services - Collaborate with interdisciplinary teams to ensure effective patient care coordination - Conduct patient assessments and develop individualized care plans - Review documentation for accuracy and compliance with regulatory standards - Coordinate discharge planning and transitions of care - Provide education and training to staff on case management best practices - Monitor and evaluate the effectiveness of case management interventions Skills: - Strong leadership abilities to effectively manage a team of case managers - In-depth knowledge of anatomy and medical terminology - Experience in behavioral health and pediatrics is preferred - Proficient in developing and implementing care plans - Familiarity with hospice care regulations and practices - Excellent communication skills for effective collaboration with healthcare professionals, patients, and families - Attention to detail for accurate documentation review - Ability to conduct comprehensive patient assessments Note: This job description is not intended to be all-inclusive.
